What is usually wrong, and what it means.

Not heating or blowing cool air: low refrigerant from a leak, a failed reversing valve, a sensor fault, or simply defrost mode in a cold snap. Steam and a paused fan in frosty weather is normal.
Ice building up on the outdoor unit: light frost is normal. A solid block means a defrost fault or an airflow problem. Shut it off and call before the fan damages itself.
Short-cycling: constant on and off from oversized equipment, low charge or a control fault. Systems that short-cycle for years die young.
Grinding, buzzing or rattling: fan motors, mounts or the compressor. Buzzing at the outdoor unit can be electrical and should not wait.
Indoor head dripping: a blocked condensate drain. A quick fix that becomes a drywall repair if left.
